    Waist-to-hip ratio divides your waist circumference by your hip circumference to show where your body stores fat — and where fat sits turns out to matter more for cardiovascular and metabolic risk than how much of it you carry. The World Health Organization treats it as a standard measure of abdominal obesity, and in several large studies it has outperformed BMI at predicting heart attack risk.

    Why fat distribution beats total weight

    Fat stored around the abdomen is metabolically different from fat stored on the hips and thighs. Visceral fat — the deep fat packed around the liver, pancreas, and intestines — is highly lipolytic, meaning it releases free fatty acids directly into the portal vein feeding the liver. That flux drives hepatic insulin resistance, elevated triglycerides, low HDL, and systemic inflammation through cytokines such as IL-6 and TNF-alpha.

    Gluteofemoral fat behaves almost oppositely.     The INTERHEART study, covering roughly 27,000 people across 52 countries, found waist-to-hip ratio was a substantially stronger predictor of myocardial infarction than BMI, with the top quintile carrying more than double the risk of the bottom quintile after adjustment. Replacing BMI with waist-to-hip ratio in risk models increased the population-attributable risk for heart attack from about 24 percent to about 50 percent.

    How to measure correctly

    Measure in the morning before eating, wearing minimal clothing, and take each measurement twice. If the two readings differ by more than half an inch, take a third and average. Consistency matters more than absolute precision because the value of the metric is largely in tracking change over time.

    A flexible non-stretch tape measure is essential — a fabric sewing tape that has stretched with age can add an inch of error. Measurement in centimetres reduces rounding error compared with inches, though the ratio itself is unit-independent so either works.

    The WHO defines abdominal obesity as a waist-to-hip ratio at or above 0.90 for men and 0.85 for women, and treats those cut points as the threshold for substantially increased metabolic complication risk. Below roughly 0.95 for men and 0.80 for women is generally regarded as low risk.

A very muscular person with wide glutes can hold a reassuring ratio while carrying a waist above those limits, which is why the calculator reports both.

    Worked example: comparing two people at the same BMI

    Two 45-year-old men are both 5-foot-10 and 190 pounds, giving each a BMI of 27.3 — the overweight category. On BMI alone they look identical.
